If you are interested in becoming an Invisalign Provider, simply complete the form below. We will contact you to provide further information to get you started on becoming an Invisalign® provider.

Invisalign® Flex program includes flexible onboarding with dedicated education and support, expert guidance from the Invisalign® Treatment Planning Service, iTero™ intraoral scanner experience and 360 practice support. Please note the fee is not refundable even if Doctor does not participate or complete the program, and which could not get the Invisalign certificate in due of that, after the fee is paid by the Doctor.
